A carer is someone who CARES, UNPAID, for a friend, family member or member of our community who:

  • is physically disabled, or

  • has a long term illness, or

  • has a learning disability, or

  • has a mental health condition, or

  • is neurodiverse, or

  • has an addiction

AND
the person they care for 
CANNOT COPE WITHOUT THEIR SUPPORT

We would like to hear from unpaid carers living in Middlesbrough!
We would like to know what support you currently receive, how you feel and how we can help you feel less isolated, improve your wellbeing and help you feel more supported in your caring role.

There are 3 questionnaires available below.
One is for adult unpaid carers of a child/children,
the next is for adult unpaid carers of an adult/adults.
The last questionnaire is for young carers age 13 - 17 years old.

Please fill out the appropriate questionnaire for your unpaid caring role.
If you are an adult unpaid carer for an adult and a child, please fill out both adult carer questionnaires.

The results of this questionnaire will be compiled, anonymised and used to hopefully secure grant funding.

Any grant funding secured will then be used to put support in place for carers and families impacted by disabilities living in Middlesbrough.
