A carer is someone who CARES, UNPAID, for a friend, family member or member of our community who:
is physically disabled, or
has a long term illness, or
has a learning disability, or
has a mental health condition, or
is neurodiverse, or
has an addiction
AND
the person they care for
CANNOT COPE WITHOUT THEIR SUPPORT

There are 3 questionnaires available below.
One is for adult unpaid carers of a child/children,
the next is for adult unpaid carers of an adult/adults.
The last questionnaire is for young carers age 13 - 17 years old.
